Please see the required Daily Student COVID Assessment Form
Any student who is reported absent or sent home ill with a symptom that is possibly related to Covid 19 must provide the results of a negative Covid PCR test prior to their return to school. No form of rapid test is acceptable.
The only exception to this would be a signed note from a health care provider indicating a known chronic condition with no change in symptoms (i.e. allergies) or a signed medical note indicating a diagnosis of acute illness that is confirmed by lab testing (i.e. influenza, strep throat). A note indicating an unconfirmed acute illness such as a cold virus or stomach virus cannot be accepted
Anytime a child is diagnosed with a significant illness or injury, parents are asked to notify the school nurse. Any student who has a fever may not return to school until he/she is fever-free for 24 hours without the use of fever-reducing medications. In the case where an antibiotic is required, the child must be on antibiotics for 24 hours before returning to school.
The school nurse may administer medications to students during the school day with a written prescription from a physician, written permission from the parent/guardian, and the medication in the original or pharmacy-labeled container.
As of January 2021, only the approved NYS School Health Examination Form may be used to document health exams to provide consistency across the state. (See link to form below.)
Public Health Law- Section 2164: All Students enrolling in any school in New York State for the first time shall submit a written statement signed by a health care provider that they have been immunized as per NYS standards.
New York State law permits schools to request a dental examination in the following grades: school entry, K, 1, 3, 5, 7. Your child may have a dental check-up during this school year to assess his/her fitness to attend school.
