Dear Parents,
Each year we print a directory of our families, also known as the Blue Book or Family Directory. We would like to include your e-mail addresses in this directory which we will send home to families who have paid their PTO dues. In order to have your e-mail published in this directory, you will need to fill out the bottom of this form and return to the School Office.
ONE E-MAIL per family, PLEASE.
PLEASE NOTE: If you do not want certain information to be in this directory, please note on the bottom of this form what information you do not want published and return to the office.
This will hopefully make correspondence easier for the Class Mom’s, Committee Heads and the P.T.O.
This needs to be in the school office as soon as possible in order to be included in the Blue Family Directory.
| Also, please be aware, the student Emergency Cards are important to the publishing of this directory. We cross check our information with the Emergency Card's information to make sure we have the most up to date addresses, telephone numbers, etc. to ensure the accuracy of this directory.
