Dear Parents,
Each year we print a directory of our families, also known as the Blue Book or Family Directory. We would like to include your Family Information and e-mail addresses in this directory which we will send home to families who have paid their PTO dues. This will hopefully make correspondence easier for the Class Mom’s, Committee Heads and the P.T.O.
In oder to have your family information published in this directory, you will need to click onto Submit Form and this will take you to the form which you need to fill out. Submit that form and we will be able to include your family information in the Family Directory. (EVEN IF YOU HAVE ALREADY DONE THIS IN THE PREVIOUS YEAR, PLEASE SUBMIT A NEW FORM.)

PLEASE NOTE: If you do not want certain information to be in this directory, please note on the bottom of the form what information you do not want published.
The E-Mail address you submit for the directory does not affect the e-mail address which I have on file for your day to day e-mails from the school office. Those e-mail addresses stay as is. If you want me to change the e-mail address the school office uses, you will need to send a seperate note to my attention.
This needs to be in the school office as soon as possible in order to be included in the Blue Family Directory.
